Fauquier Health Rehabilitation & Nursing Center (FHRNC) is a 115-bed center located on the Fauquier Hospital campus in Northern Virginia. Our convenient access to hospital and physician services allows for a rare continuity of care not found at other institutions.
Commission on Accreditation of Rehabilitation Facilities (CARF)
Our QualityThe center has earned accreditation for inpatient rehabilitation programs – skilled nursing (adults), and for person-centered long-term care community (older adults). This coveted accreditation comes from CARF, the Commission on Accreditation of Rehabilitation Facilities.
The 2008 CARF accreditation put FHRNC at the top of its field as one of only eight facilities in the United States with a person-centered long-term care designation and one of only three facilities in Virginia with inpatient rehab. It is the only facility in the state with both.
After a detailed site visit, the Commission on Accreditation of Rehabilitation Facilities review team reported on Fauquier Health Rehabilitation & Nursing Center:
“The leadership for the program is experienced, knowledgeable and dedicated to pursuing excellence in service delivery. The organization has a committed, enthusiastic group of well-qualified employees who have proven they are capable of carrying out the mission of the organization and are excited about further developing the potential of the programs seeking accreditation. Dignity and respect are accorded to all persons served and to staff members. This is consistently demonstrated in the interactions between staff members and persons served.”
Our Philosophy
Our center participates and is fully committed to the Planetree patient-centered care alliance. The Planetree philosophy is a holistic approach to patient care that encompasses the body, mind and spirit. It embraces the participation of family and friends in care and rehabilitation, and education is another Planetree cornerstone that insures that both the patient and family understand any medical diagnosis and treatment plans.
As a Planetree affiliate we respect the needs and preferences of our residents and their families and friends. Special emphasis is placed on the importance of the nutritional and nurturing aspects of food, so our patients get what they need as well as what they like. Meal times are flexible, and visitation is not restricted to certain hours of the day.
Regular visits from our pet therapy animals or bonding with one of the animals that live on-site are also very therapeutic for our patients.
Our Services
Our Short-Term Rehabilitation Services: Once discharged from the hospital after a surgical procedure like joint replacement or a medical condition such as stroke, you may not be ready to return to your normal activities. Our center can provide short-term rehabilitation as the transition you need to regain your strength and flexibility in a caring, home-like environment. FHRNC specializes in orthopedic conditions, neurological disorders and wound management, providing the next level of recovery services after you leave the hospital.
Our Facility: Our rehabilitative services feature modern therapy equipment and the only indoor rehabilitative therapy pool in the region. Staffed by specialty trained therapists, it features a hydraulic lift for patients with mobility limitations.
Our Therapists: Our physical therapists, occupational therapists and speech pathologist will work with you to develop a rehabilitation plan that matches your personal needs so that you will feel comfortable returning home to once again take on the activities of you everyday life. We’re in the business of getting you back to being your best self again.

Long-Term Care: For many, the center is a positive new beginning, an opportunity to live in the company of others, secure in the knowledge that their medical needs will be met. However, for some, long-term care can be a difficult change. We understand and work to ease that transition with compassion, kindness and professionalism.
It is our mission to meet the physical, emotional, spiritual and intellectual needs of the whole person. We understand that, for our long-term care residents, this is their home and we want to insure that they feel comfortable.
A busy schedule full of activities adds structure and security to one’s life. To keep our residents active and involved, the center plans hundreds of enriching activities each year, including frequent community outings in our custom touring van. We also encourage physical and spiritual health through exercise programs, religious services and other activities.
We also understand the importance of just sitting and chatting. Sometimes caring companionship means more than anything, and we provide that too.
Respite Care
Whether you are going out of town for the weekend or just need some time to yourself, our respite care is designed to offer you much needed short-term breaks while providing a safe and comfortable haven for your loved one. You can relax and recharge knowing that their physical and medical needs are being met, and that they are being treated with kindness and understanding.
Our Medical Staff
Our FHRNC staff includes board-certified physicians, registered and licensed practical nurses, certified nursing assistants and licensed physical therapists. Their skill and compassion means you or your loved one is in good hands 24 hours a day, 7 days a week. There are always competent medical personnel on duty should the need arise.
Our Environment
Our center is bright, safe, clean and well maintained. Situated on a hill overlooking the town of Warrenton, we are surrounded by rural beauty and the simplicity of small town life. All of our efforts are designed to make all who enter feel comfortable.
Family and friend visits are not restricted to certain hours because we understand that life doesn’t happen on a time clock. We encourage family and friends to join us for meals. We work diligently to develop a positive and meaningful relationship with each of our residents and their families.
Understanding Your Medical Coverage
The center is a “Medicare/Medicaid Certified” facility. This means that we offer a full range of services that are approved and covered by Medicare and most other insurance carriers. Understanding coverage regulations can be time consuming and difficult. Fortunately, we know how to work within the complicated guidelines, as well as how to work with physicians most effectively.
